The AIDS Foundation of Chicago (AFC) mobilizes communities to create equity and justice for people living with and vulnerable to HIV or chronic conditions. We envision a world in which people living with HIV or chronic conditions will thrive, and there will be no new HIV cases. Our work focuses on advancing health equity, preventing new cases of HIV, serving as a collaboration and knowledge center, being a bold voice for change and strengthening our organizational excellence.

In order to achieve our goals, we are seeking for an individual to serve as our Program Manager. The Program Manager will oversee programs related to behavioral health services. This includes the Road to Health and Housing (R2H), a five-year grant awarded from SAMHSA and AFC’s in-home behavioral health program. R2H project will increase the availability and ensure coordination of integrated behavioral health, primary care, housing and supportive services to improve physical health, behavioral health and housing stability for homeless adults with Serious Mental Illness (SMI) or Co-occurring Disorders (COD) in the city of Chicago. This position will manage and supervise the day-to-day operations of the project, providing clinical supervision as well as work across sectors including homeless services, health care and legal advocacy organizations.  In addition, Program Manager will ensure access to mental health services by providing therapy to a small caseload of supportive housing clients.

Principal functions and responsibilities of this position include the following:

Program Management

  • Manage relationships with partner agencies including housing, homeless services, and healthcare providers;
  • Promote the Trauma Informed Care approach within the continuum of housing and healthcare providers who work with people experiencing homelessness;
  • Support the development and achievement of team goals and related strategic plan items;
  • Responsible for Behavioral Health Program’s daily operations;
  • Ensure program policies and procedures are implemented and followed;
  • Coordinate case conferencing between project staff;
  • Provide weekly clinical supervision to Behavioral Health Specialist (1) and R2H Clinical Engagement Specialists (3).

Staff Supervision

  • Hire, train, mentor, and supervise assigned staff; this includes assisting with recruitment and selection activities, scheduling and job assignments, mentoring/coaching, staff development and training, and performance management;
  • Meet regularly with assigned staff to identify priorities, discuss strategies, and review ongoing projects;
  • Provide professional development opportunities and guidance to each team member to strengthen their skill sets and job satisfaction.

Project Coordination

  • Coordinate projects serving households experiencing homelessness with behavioral health needs;
  • Serve as a liaison to partner agencies including All Chicago, Heartland Alliance Health and Legal Council for Health Justice to coordinate housing, behavioral health, health and wraparound services;
  • Utilize data for planning purposes and for system improvement;
  • Ensure a streamlined process to obtain referrals for Behavioral Health Programs and work with partner agencies to locate, assess for eligibility, and house clients in a timely manner;
  • Monitors staff compliance with reporting in information systems including HMIS and Caseworthy;
  • Create and maintain detailed project documentation and data collection plans and reports;

Quality Assurance and Data Entry

  • Complete data entry in information systems (i.e., HMIS, Caseworthy) as needed;
  • Analyze project data to provide quality and statistical reports to stakeholders;
  • Serve as liaison to Research and Evaluation team and Data Services team to ensure that data is being collected in timely and accurate manner.

Meetings and Training Facilitation

  • Attend required conference, trainings, and webinars;
  • Facilitate partnership meetings with stakeholders and leaders at partnering agencies;
  • Train service providers on Trauma Informed Care and Behavioral Health Programs;
  • Deliver presentations to key stakeholder groups;
  • Educate and train health care providers on working with people experiencing homelessness;
  • Convene meetings with project/program stakeholders to establish work plans and further initiatives;
  • Provide subject-matter guidance and mentorship to other department professionals.

Clinical Practice

  • Interview and evaluate the behavioral health needs of program participants in their home by using a variety of assessment techniques and instruments; assess, and re-assess behavioral health needs of program participants;
  • Develop individualized treatment plans for each program participant and update plans every six months;
  • Provide intensive individual counseling to program participants at their level of understanding and acceptance;
  • Conduct clinical assessments and provide counseling to clients when necessary;
  • Provide crisis intervention services to clients;
  • Coordinate referrals for behavioral health treatment; act as a liaison to community treatment programs;
  • Facilitate communication with client’s Medicaid managed care health plan, care coordinator, primary care provider and/or psychiatrist to meet integrated health needs;
  • Conduct and document intake assessments, program discharges, and referrals;
  • Schedule all client visits; make reminder calls and organize electronic and hard copy calendar;
  • Document assessment information as required in the program participants’ files; input information into data tracking system and on paper charts; ensure quality of counseling files.

Other

  • Assist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others;
  • Update job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations;
  • Protect organization's value and manage risk by keeping information confidential;
  • Perform other duties as assigned.

QUALIFICATIONS

Minimum Qualifications

Master’s degree in Psychology, Social Work, Counseling or related field

LSW/LPC, LCPC/LCSW or equivalent license

1 or more years Case Management experience

1 or more years Supervisory experience

Preferred Qualifications

1 or more years Homeless Services experience

Knowledge, Skills, and Abilities

Strong attention to detail and the ability to manage multiple projects, establish workload priorities, and meet deadlines are critical. 

The ability to train, coach, and develop employees. 

The ability to make reasoned judgments that are logical and well thought out.

The ability to breakdown complex processes to identify quality improvements and put measurements in place.

Strong case management and the ability to assess client needs create care plans, and follow-up in order to address barriers and ensure care is continuous and comprehensive.

Also, strong verbal and written communication skills and ability to work with diverse populations are key to success in this role.
