We are seeking for an individual to serve as our Coordinated Entry Specialist. The Coordinated Entry Specialist will ensure coordination of outreach and assessment services through the Coordinated Entry System (CES) for households that experience homelessness in Chicago. The position will assist in building collaboration between partners, including outreach and housing providers, to assess households through the process of assessment and connection with housing partners. The Coordinated Entry Specialist will also coordinate Access Points and will support communication regarding Access Points to the broader community. Center for Housing and Health (CHH) is a supporting organization of the AIDS Foundation of Chicago.
Principal functions and responsibilities of this position include the following:
- Assist in coordination and monitoring of CES Access Points;
- Work with agencies that have Skilled Assessors on-staff to identify appropriate assessment workflow and troubleshoot assessment processes;
- Conduct site visits to assessment sites;
- Provide regular reports to outreach, shelters, and Access Points about their assessment progress;
- Use data to ensure consistent assessment completion;
- Participate in weekly CES Implementation Team meetings and complete follow up work related to CES Action Plan in collaboration with other CES lead agencies;
- Assist with preparation for system-wide System Integration Team meetings;
- Develop and refine materials for Skilled Assessors, such as scripts and materials for consumers;
- Serve as a point person for Skilled Assessors and outreach to troubleshoot the assessment and navigation processes for people matched to housing;
- Serve as the first person of contact for inquiries and concerns of providers and CES applicants and bring their feedback to the CES Implementation Team to improve the system;
- Assist with assessment surges for specific populations to meet the system’s objectives;
- Monitor and serve as the first person of contact for inquiries on CES matching emails including connecting households to appropriate resources and facilitating communication between providers;
- In collaboration with the Program Manager, make recommendations based on federal, state, and local policies and procedures for CHH and Chicago Continuum of Care (CoC) programs;
- Create and maintain detailed project documentation and data collection plans and reports;
- Serve as a CES Skilled Assessor to assess people experiencing homelessness for housing;
- Serve as point of contact to others within CHH regarding CES.
Quality Assurance and Data Entry
- Enter all intake data for new clients in client data tracking systems (i.e., Client Track, HMIS);
- Enter all close-out data on clients;
- Monitor data for quality control, correct missing or incorrect data on quarterly (i.e., IDHS, HMIS) and yearly reports (i.e., APR HUD);
- Complete data entry in HMIS, ClientTrack, etc. as needed.
Meetings & Training Facilitation
- Assist with preparation for Systems Integration Team (SIT) meetings for special populations to ensure adequate assessment coverage for CES and that people reach housing;
- In collaboration with the Program Manager, co-facilitate partnership meetings with stakeholders;
- Attend required conferences, trainings, and webinars;
- Provide regular CES trainings and webinars for Skilled Assessors;
- Offer presentations on CES to key stakeholder groups including educating external systems on integrating with CES.
- Assist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others;
- Update job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations;
- Protect organization's value and manage risk by keeping information confidential;
- Perform other duties as assigned.
Bachelor's degree and 2 or more years of Human Services experience
OR 5 or more years Human Services experience
Master’s degree in Psychology, Social Work, Counseling or related field
1 or more years Case Management experience
1 or more years Homeless Services experience
Knowledge, Skills, and Abilities
Exceptionally strong written and verbal communication skills, coupled with presentation skills.
Strong attention to detail and the ability to manage multiple projects, establish workload priorities, and meet deadlines.
Solid interpersonal skills along with the ability and willingness to respect and value the differences and perceptions of different groups/individuals.
Knowledge of the causes of homelessness, who it affects, and the factors (e.g., racial inequities, Socio Economic Status) that can contribute to an individual being homeless.
