We are seeking an individual to serve as our Senior Services Case Manager. The Senior Services Case Manager range of client-centered, confidential services that link older adults living with HIV/AIDS with core care services including: medical, dental, mental health, substance use, and supportive care services including legal, medical transportation, emergency financial assistance, food and housing services. The role of the Senior Services case manager is to serve as a subject matter expert in assessments and service delivery to older adults living with HIV.   The goal is to ensure viral suppression by taking care of any general barriers that are unique to older adults over the age of 50 that may be preventing a client from being undetectable. The Senior Services Case Manager will provide treatment adherence counseling to ensure readiness for, and adherence to complex HIV/AIDS treatments. The Senior Services Case Manager will screen and enroll eligible clients in the 340B program.

The Senior Services Case Manager will recruit clients from various sources including the Case Management Collaborative. They will also participate in community coalitions on HIV and aging.

Principle functions and responsibilities of this position include the following:

Assessments & Service Plans

  • Conduct assessments and reassessments every six months on Older Adults Living with HIV;
  • Create care plans that match the identified needs of the client;
  • Ensure that all clients referred to services are eligible prior to the date of service.

Client Service

  • Coordinate care provision with any other case managers, for example Ryan White, Corrections, DRS, Mother and Child Alliance;
  • Provide care services linkage and re-engagement support to clients;
  • Conduct field visits as needed to clients identified as having disengaged from care; or clients in need of in-home safety assessments
  • Accompany clients to their medical visits as needed;
  • Support with behavioral health challenges, chronic illness management, and medication management
  • Conduct outreach services as needed;
  • Refer clients to appropriate services highlighted in the care plan;
  • Screen for income supports for benefits and entitlements, especially those that are available to older adults/seniors to support the client in applying and advocating on behalf of client;
  • Complete emergency housing applications for eligible clients;
  • Complete payment requests as needed for copays and other out of pocket costs;
  • Document encounters using designated client database accurately and in a timely manner;
  • Develop and maintains professional relationships with social service, health and governmental agencies;
  • Participate in continuous quality improvement efforts;
  • Participate in regular supervision meetings, team meetings, in house training sessions (12 sessions per year with no less than 4 being specific to older adult/senior services) and conferences as needed.


  • Attend and actively participate in required educational programs;
  • Update job knowledge by participating in educational opportunities; this includes reading professional publications, maintaining personal network, and participating in professional organizations;
  • Serve as the agency subject matter expert on issues relevant to Older Adults Living with HIV;
  • Work collaboratively with the Director of Special Projects and Planning to coordinate and plan for services for Older Adults Living with HIV;
  • Assist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others;
  • Protect organization's value and manage risk by keeping information confidential;
  • Perform other duties as assigned.


Minimum Qualifications

Associate’s degree in the Human Services field (i.e., Psychology, Sociology, Public Health, Social Work) with at least 3 years of experience in serving older adults or working in the field of senior services

Preferred Qualifications

Master’s/Bachelor’s degree in the Human Services field (i.e., Psychology, Sociology, Public Health, Social Work) with 5 or more years of experience in serving older adults or working in the field of senior services

3 or more years of experience using the Provide database


Exceptionally verbal communication and active listening skills.

Strong attention to detail and the ability to provide efficient, quality service to both internal and external customers.

Solid administrative skills and knowledge of Microsoft Excel and/or other database programs.

Solid interpersonal skills along with the ability and willingness to respect and value the differences and perceptions of different groups/individuals. 

Strong case management skills with basic knowledge of HIV/AIDS and understanding of HIPPA.

Solid adaptability/flexibility skills and ability to follow up with clients in a timely manner.

Strong time management skills along with problem solving skills and ability to quickly and accurately process multiple types of information and/or perform mutable tasks simultaneously.
