Case Management Generalist

Job Type: Full-Time | Temporary with Temp-to-Hire Potential
Pay: $49,000-$58,000 annually

Monarch Staffing is partnering with a respected nonprofit organization in Delaware County to hire a Case Management Generalist / Floater. This position is ideal for an experienced human services professional who enjoys variety, thrives in a collaborative environment, and is passionate about helping individuals and families overcome barriers to stability.

You'll support multiple programs and departments, providing critical case management services wherever they're needed most.

Responsibilities

• Conduct client intake, assessments, and eligibility screenings

• Develop and monitor individualized service plans

• Maintain regular client contact, advocacy, and follow-up

• Connect clients with community resources and support services

• Maintain accurate documentation, case notes, and electronic records

• Assist with referrals and service coordination

• Support multiple departments based on operational needs

• Provide temporary coverage for various programs throughout the agency

• Ensure compliance with funding and documentation requirements

Qualifications

• Bachelor's degree required (Human Services, Social Work, Psychology, Sociology, Counseling, or related field preferred)

• Three or more years of case management or human services experience preferred

• Strong communication, organization, and documentation skills

• Experience working with diverse populations facing economic hardship, housing instability, or other barriers

• HMIS experience is a plus

• Ability to work independently while collaborating with multidisciplinary teams

Important Requirements

• Reliable transportation is required.

• This is a fully in-person position.

• Travel between agency locations throughout Delaware County, including Boothwyn, Chester, Darby, Upper Darby, and surrounding areas, is required.

Schedule

• Monday-Friday

• 8:00 AM-4:30 PM or 8:30 AM-5:00 PM
