Broad Statement of Responsibilities: As the clinical Manager provides clinical services, managerial supervision, case conceptualization at APN Lodge. The individual in this role is responsible for the overall quality of care, outcomes management, staffing and service levels within the assigned clinical team. Ensures appropriate and effective staffing levels for all clinical programming including groups and individual sessions. Champions, with operations, compliance and revenue cycle management at APN, enhances quality and ensures adherence to service level standards and outcomes. Assists in program development at the discretion of the Chief Clinical Officer and Clinical Director. This individual provides individual and group counseling services to patients and families while leading an Interdisciplinary team which develops individualized treatment plans for each patient.

Education: This position requires CO licensure, a Graduate Degree in Social Work, Counseling, or Psychology from an accredited college or university. Ideally, will possess Dual Licensure: Colorado OBH credentialing (CACIII)\ licensing (LAC) in addiction counseling and OBH mental health professional licensure (or equivalent and ability to acquire equitable Colorado credentials). Experience: Five years clinical and three years supervisory experience working with clients in SA\MH settings. Must demonstrate working knowledge of clinical supervision with behavioral healthcare professionals and a documented history of facilitating clinical supervision for candidates seeking or maintaining professional credentials. Experience also to include family, group, and individual therapy/counseling.

Position Duties and Responsibilities – Responsibilities include but are not limited to:
- Oversee Clinical delivery and continuum of care step-downs for the assigned treatment, including RTC/ PHP/TIOP/TOP Programs
- Host weekly APN Way meetings for team and ensure collaboration in continuum of care commitments.
- Oversee and manage all elements of the client experience for all for the team
- Collaborate with Referral Sources and ensure proper communication

SUPERVISION & ADMINSTRATIVE DUTIES
- Provide effective role modeling and leadership to the treatment team, including weekly supervision
for therapists\addiction\mental health counselors - Assist in completing 90-day and annual performance evaluations per established timelines when
indicated. - Ensure coverage for all scheduled services.
- Foster a positive, collaborative, and communicative team environment.
- Ensuring staff adheres to all policies and procedures
- Timecard management, including approval and denial PTO and sick leave. Managing and tracking PTO
to ensure no more than 2 clinicians are out on PTO at the same time. Finding coverage for your team
if they fail to do so. - Ensuring all staff schedules are accurate and up to date in the centralized scheduling system
and update schedules and adjust with new additions to the team.
1. Columbia-Suicide Severity Rating Scale
- Ensure that all client charts include the Columbia-Suicide Severity Rating Scale (CSSRS)
assessments as appropriate. - Confirm that Brown Stanley Safety Plans are completed and included in client charts when
necessary. - Ensure therapists complete these assessments and safety plans within the required time frame
and documented accurately within each chart.
2. BIO-PSYCHO-SOCIAL (BPS) ASSESSMENTS
- Ensure the completion of Biopsychosocial Assessments within 72 hours of client admission with the
admissions therapist. - Confirm assessments are signed by both the admission therapist and Tami Romano.
- Oversee that if the admission therapist is unavailable, that the assigned primary therapist completes
the assessment promptly and within the 72-hour timeframe from admission. - If the primary therapist cannot complete the BPS, provide support within the timeframe.
- Verify that the BPS is accurately documented and signed within the required timeframe.
- Follow TJC, CARF and All Points North Lodge CQI processes.
3. DIAGNOSTIC SUMMARIES.- Collect pertinent data.
- Produce diagnostic summaries in a timely manner.
- Follow TJC, CARF and All Points North Lodge CQI standards.
4. GOLDEN THREAD PROBLEM LISTS, GOLDEN THREAD TREATMENT PLANS, AND TREATMENT PLAN REVIEW FORMS
- Ensure the Golden Thread Problem List includes at least 3 problems identified from the
Biopsychosocial Assessment. - Verify that the Problem List is completed within 72 hours of client admission with appropriate
signatures. - Ensure that Golden Thread Treatment Plans correspond to the identified problems in the Golden
Thread Problem List, with a minimum of 3 treatment plans, for your team. - Ensure that these treatment plans are completed within 72 hours of client admission.
- Confirm that treatment plans are signed by the client, provider, and therapist, with all appropriate
signatures in place and within that 72-hour timeframe. - Ensure the team devises treatment plans that reflect current evidence-based practices
- Ensure therapists are completing Treatment Plan Review Forms, every seven days, throughout the
client's length of stay. - Ensure appropriate client and staff signature on the Treatment Plan Review Forms within the
timeframe. - Follow TJC, CARF, OBH and All Points North Lodge CQI standards.
5. DISCHARGE PLANNING
- Ensure therapists are participating in the APN Way Continuum of Care Call.
- Verify that therapists communicate appropriately with Intensive Case Managers (ICAs), Family
Therapy, Telehealth, Alumni Services, and other care team colleagues. - Ensure that if therapists cannot attend the Continuum of Care Call, they communicate to the
appropriate people ahead of the meeting what needs to be discussed. - Ensure that discharge planning documentation is completed accurately in each client chart so that
other team members can clearly see the progress. - Confirm that anticipated discharge dates are accurate.
- Ensure discharge summaries are completed within 5 days of discharge.
- Ensure that discharge criteria forms, if needed, are completed within 5 days of discharge.
- Formulate appropriate post-discharge placement plans that include services for client and family that
maximize potential for success and include recommendations and referrals to all relevant services. - Inform administrative staff of pending/planned discharges.
- At discharge, assess consumer’s appropriateness for discharge and continued care needs.
6. FAMILY THERAPY
- Ensure clients are attending family therapy consults once scheduled at admission.
- Ensure that clients do not miss family therapy appointments.
- Verify that therapists are communicating appropriately with family therapists.
- Ensure that documentation for each client is in the chart for family therapists to refer to.
- Address any issues promptly.
7. INDIVIDUAL THERAPY- Provide therapy to clients as is deemed needed by treatment plans.
- Ensure that each client is met with for individual therapy sessions twice a week, regardless of the level
of care, until a 30-day length of stay is reached, which is then once a week. - Verify that documentation for each individual therapy session is completed within 24 hours of the session.
- Monitor and ensure therapists adhere to these requirements and provide timely and appropriate care.
- Use and accept co-counselor(s) for observation/supervision and training.
8. GROUP THERAPY
- Provide therapy to clients on a group basis as is deemed necessary by treatment plans.
- Organize and conduct weekly specialty groups as assigned.
- Use and accept co-counselor(s) for observation/supervision and training.
9. TREATMENT TEAM
- Facilitate regularly scheduled treatment team meetings.
- Provide clinical expertise and interact with multi-disciplinary treatment team.
- Document meetings and recommendations within the client medical record.
10. CONTACT WITH REFERRAL SOURCES
- Maintain regular contact with referral sources as indicated.
- Update on client’s progress.
- Develop appropriate discharge plans collaborating with referral sources.
- Prepare and conduct concurrent reviews to referral sources.
11. DOCUMENTATION
- Conduct regular chart audits to ensure compliance with all relevant laws, regulations, and accreditation standards (e.g., JCAHO).
- Conduct weekly chart audits on therapists to ensure compliance.
- Ensure that all documentation is completed accurately and in a timely manner.
- Monitor and address any documentation issues promptly to ensure high-quality clinical records.
- Responsible for timely completion of group notes within 24 hours.
12. CQI & TEAMS
- Attend and participate in one or more CQI teams annually.
- Participate in peer record review process.
- OBH data entry where indicated.
13. STAFF RELATIONSHIPS
- Maintain productive, effective and professional relationships with all disciplines.
- Keeps supervisor informed.
- Open to feedback and supervision.
