EHCA is seeking a Community Home Coordinator to provide overall direct/personal care and program services for the Agency’s clients. This is for a full-time job opportunity in Erie PA
The Community Home Coordinator will implement individualized programs following written objectives and plans and record all necessary information, observations, and daily progress notes. Plans and implements various social, recreational, work, shopping and meal preparation activities are also required. Help and support the clients in coping with day-to-day activities. Management of the house as directed.
The Community Home Coordinator will also be responsible for the care and upkeep of facilities, monitoring Medical supply orders and grocery shopping for the facility. Providing transportation to our clients in EHCA and personal vehicles may also be required. Oversees cooking and shopping with individuals, and provides training in such skills. Communicates with therapists as needed. Communicates with families regarding any activities. Perform weekly Safety checks.
Direct Support Professional Job Requirements:
- High school diploma or general education degree (GED).
- 18 years of age or older
- Valid Drivers License
- Some travel required
- Able to lift and/or move up to 50 lbs.
- Immediate supervision of shifts as assigned
- Complete First Aid/CPR training
- Knowledge of basic practices and techniques for persons with developmental disabilities.
