EHCA is seeking a Program Specialist Professional to provide overall direct/personal care and program services for the Agency’s clients. This is for a full-time job opportunity in Erie, PA. 40 HOURS PER WEEK (MUST BE FLEXIBLE)
The Program Specialist plans, organizes, and directs the provision of cross-disciplinary services to clients in a home-based program for intellectually disabled. Will act as team facilitator in Interdisciplinary Team meetings to develop Individual Service Plans (ISP) on each assigned client. Complete monthly, quarterly, semi-annual, and annual reports on the progress of the Individual Service Plan for each client and furnish written reports on assigned residents to designated parties as outlined. Maintain ongoing staff training in all components of the Goal Planning/Programming. This will include formal training sessions and on-site training.
Coordinate and implement an appropriate educational/recreational program in the daily routine of the clients. Function as a liaison between parents, county personnel, educational institutions and EHCA. Participate in all meetings that are related to the client and or their care. Will be responsible for directly supervising Direct Support Professionals and/or support staff. Approve all programmatic areas for clients. This includes ISP’s, goal plans, programs, data collection systems, informal guidelines and any other intervention that is needed.
Program Specialist Job Requirements:
- An associate’s degree (or higher) or at least 60 credit hours from an accredited college or university and 4 years work experience working with persons with intellectual disabilities.
- 18 years of age or older
- Valid Drivers License
- Can lift and/or move up to 50 pounds.
- Complete First Aid/CPR training
- Some travel is required
