Position Summary: 
 The Out-of-Home Care Projects Specialist serves as a centralized liaison between Information & Eligibility and placement operations, supporting accurate placement records, timely coordination, and efficient service delivery across assigned counties. This role reconciles data across agency systems, (Visual Vault, Argos, FSFN/CCWIS, CCIS), identifies trends and service gaps, supports project tracking, and provides flexible coverage for key Out-of-Home Care functions based on departmental need.
 
Essential Functions:
 
1. Data Reconciliation and Placement Record Accuracy:
Serve as a centralized point of contact for reconciling placement-related data across agency systems, including Visual Vault, Argos, and FSFN/CCWIS. Investigate discrepancies involving placement dates, times, locations, and related documentation to support accurate and timely child placement records.
2. Project Tracking and Trend Analysis:
Collect, organize, and analyze data for assigned projects; identify trends, service gaps, and operational barriers; and support recommendations for process improvement within Out-of-Home Care operations.
3. Placement and Intake Support Coverage:
Provide needs-based coverage for Out-of-Home Care Placement Specialists and Intake Behavioral Health Coordinators to maintain continuity of service during vacancies, absences, peak workload periods, or other departmental needs.
4. On-Call Support:
Participate in the second-tier on-call rotation to support timely placement coordination and service delivery outside standard business hours, as assigned.
5. Family, Caregiver, and Partner Support:
Communicate professionally with children, families, caregivers, providers, and internal partners to support problem solving, stability, and appropriate service coordination.
6. Training, Special Projects, and Additional Support:
Assist with training, special projects, data requests, and other assigned responsibilities that support Out-of-Home Care operations. Provide temporary coverage for related functions as needed and within role expectations.

Qualifications
 
Education
1. Bachelor&rsquo;s degree in social work, human services, behavioral health, public administration, or a related field. (required)
2. Master&rsquo;s degree in a related field (preferred)
Experience
1. 2&ndash;5 years related child welfare experience with an emphasis on placement in licensed out of home care (required)
2.  5&ndash;8 years related child welfare experience with an emphasis on placement in licensed out of home care (preferred)

Position Summary: The Family Allies OCS/ICPC Care Manager adheres to the FPOCF System of Care principles of practice and provides family cantered strength-based assessment, planning, linkage, coordination, and advocacy for children and families. This position provides direct support to children and parents whose primary case is out of county but the agency is providing Out of County Supervision or Case Plan Assistance as well as ICPC cases.  The OCS/ICPC Care Manager will also receive all incoming OCS/ICPC requests and will act as the liaison for all Care Managers on out of state ICPC needs.  The OCS/ICPC Care Manager works as a team member with key partners including parents, family members, foster parents, caregivers, guardian ad litem, DCF Protective Investigators, CLS attorneys, therapists, providers, and other appropriate professionals. The OCS/ICPC Care Manager will train and become proficient in identifying and assessing child and family needs with the ultimate goal of achieving permanency. 
 
Must successfully complete Child Protection Certification training and examination, then a considerable period of monitored field practice under close supervision and the associated field case assessment process. 
 
Technical/Functional Expectations:
 
1.       Complete required certification - Attend Child Protection Certification training (pre-service) and successfully attain a passing score of 78% or higher on the required examination.
2.       Safety - Conduct family function and child safety assessments; develop, monitor, and manage required safety plans.
3.       Permanency - Identify and develop plan to successfully achieve permanency for children placed in out-of-home care. 
4.       Service Delivery - Assess child and family needs; develop service plans; coordinate the delivery of services; ensure all referrals and linkages to children/parents/caregivers are completed. 
5.       Reports - Prepare all required court reports and submit within the required timeframes; make recommendations; testify at court hearings regarding reunification, permanent guardianship, termination of parental rights, or other case management activities; ensure court orders are completed within required timeframes.
6.       Documentation &ndash; Develop and maintain case and program documentation according to contract; ensure documentation occurs within the SACWIS system and other related systems within required timeframes.
7.       Engagement - Adherence to the local SOC values, goals and priorities for Brevard&rsquo;s children, youth and families; Utilize a family centered strength based approach to care; Monitor quality of care by conducting face-to-face home visits with children minimally every 30 days, or with increased frequency as determined by case situations and supervisory direction; facilitate, monitor, and assess visitation between children, parents, and siblings to ensure safety and observe interactions.
8.       Support - Provide support to children, caregivers and family members to assure stable placement and timely achievement of permanency; Coordinate cases with supervisor, team members, providers, DCF and attorneys as needed.
9.       Court Appearance and Preparation- Review case information prior to every hearing in order to be prepared to answer any questions from the Court regarding the status of the case, services, and progress towards permanency; Dress appropriately for Court appearances and ensure all court hearings are attended timely. 
10.     Initiatives: Embraces and participates in initiatives and programs designed to enhance the System of Care. 
11.     On Call:  Participate in unit on call rotation for dependency case management.

Position Summary: The Program Integrity Manager serves as the credential verification subject matter expert for the FPOCF System of Care Provider Network. This position is responsible for screening prospective providers before contract or rate agreement execution, verifying provider credentials and legitimacy, and conducting ongoing monitoring of existing providers. This includes cross-walking provider staff qualifications and third-party provider qualifications against the menu of clinical, behavioral health, mental health, and substance abuse treatment services to verify required credentials, relevant degree or education, licensure, and specialty certifications needed to deliver high-quality services to children and families. The Program Integrity Manager conducts ongoing audits, collaborates with the Contracts and Compliance department, reports to the Senior Director of Provider and Stakeholder Partnerships, and participates in the third tier on-call rotation.
 
Technical Functions include:
1.       Credentialing Verification Subject Matter Expert- Verifies and monitors provider credentials, relevant degrees or education, specialty certifications, and legitimacy of service providers in the FPOCF System of Care Provider Network.
2.       Screening of New Providers/Ongoing Monitoring and Audits of Existing Providers- Engages in proactive provider screening  of new providers to ensure the required credentials, licensure and training to effectively deliver specific contacted service. Continuous review of professional&rsquo;s credentials, background, and qualification of current contracted providers. Tracks real-time changes to prevent safety risks.
3.       Florida State Medicaid Managed Care Provider Search- Utilizes Florida State Medicaid Managed Care Provider Search to check providers and specialists participating in state programs.
4.       Licensing Verification-Utilizes the Florida Department of Health Medical Quality Assurance Search for license verification and practitioner profile.
5.       Interface with the Managing Entity- Collaborates with non-profit corporation contracted by the state to oversee local mental health and substance abuse services to check provider status and review provider performance.
6.       Agency Representative Provider Concerns-Lead on any reported concerns in activity or intentional deception or misrepresentation from clinical, behavioral and substance abuse providers.
7.       Programmatic Compliance-Ensures compliance with all regulatory, accreditation, and contractual requirements, adheres to all applicable regulatory, accreditation, and professional ethics standard.

Position Summary:
The Peer Support Advocate is part of the Seminole Behavioral Health Expansion (SBHE) project funded by Substance Abuse Mental Health Service Administration (SAMHSA). The intent of this position is to increase family and youth voice and choice within the system of care, representing cultural and linguistic competency, specifically, but not limited to those who have been identified as on the neurodiversity spectrum. This will be done by building community partnerships, expanding cultural alliances, recruiting, facilitating supportive activities, and training peer (family/youth) advocates, and assisting organizations to incorporate youth and family voice at multiple levels. This position will also provide consultation with the Wraparound training and Replication Teams. This position is a 4-year federal grant funded position.
 
 Essential Duties and Responsibilities:

Build partnerships with community stakeholders, child serving agencies, and community partners to value and invest in family and youth involvement activities.
Participate in the development, implementation, and monitoring of the SBHE strategic plan specifically in goals and objectives focused on family and youth involvement.
Recruit and support diverse family and youth voice/participation representing the cultural and linguistic backgrounds of the populations of focus in strategic planning process; specifically, but not limited to those who have been identified as on the neurodiversity spectrum.
Coordinate and facilitate ongoing training and technical assistance to build family and youth leadership, build family/youth and professional partnerships, and implement family-driven/youth guided practices.
      Acquire the Peer Support Certification through the state of Florida.
Help develop infrastructure and activities to create, develop, and build up a community of resources representing the cultural and linguistic background of the populations of focus that will support and promote family and youth involvement and the family/youth voice at all system levels.Coordinate social media activities and assist with the expansion of virtual interaction with families and youth.

 
Qualifications:

 Personal &ldquo;lived&rdquo; experience representing Family/Youth Voice in at least one of the following systems (Child Welfare, Mental Health, and/or another mandated system) i.e. parent or caregiver of a child (or children) or adolescent with a primary mental health need and has received or is currently receiving services for these needs. Youth that has experienced the foster care system and/or has or is living with a mental health challenge.
 High school diploma and at least 5 years&rsquo; experience of professional advocacy or social service experience. -OR-


BA/BS - bachelor&rsquo;s degree preferred with at least 2 years of professional advocacy or social service experience.
     3. Must have experience navigating the Child Welfare System of Care or other formal systems (i.e. Juvenile           Justice, Education, Mental Health)
     4. Demonstrate cultural and linguistic competence and sensitivity to population served.
Skills/Knowledge/Experience/Personal Qualities Required:

Must be exceptionally well organized and be capable of completing assignments with minimum guidance and supervision.

3.       Utilization Management--Essential Function: Authorization and monitoring of resources, including agency and community, to meet client&rsquo;s needs.

This position will also provide consultation with the Wraparound training and Replication Teams. The Lead Family Coordinator will also provide linkage to community support groups for parents and children identified on the neurodiversity spectrum. This position is a 4-year federal grant funded position.
 
Technical/Functional Expectations:
1.       Assist with Monitoring Program Performance/ Adherence to Contract Measures &ndash; essential function: This position works alongside the Project Director to monitor performance in accordance with contractual measures and provides strategic direction to produce desired outcomes.
 2.       Strength Based Family/Youth Centered Practice Orientation &ndash; essential function: This position is expected to work in a Strength Based Family Centered manner to enhance Agency Wraparound philosophy and values. This position will participate in Trauma Informed Care initiatives.

The Project Director is a member of the SBHE grant management team and will work collaboratively to implement the strategic plan. 1.       Assist with Monitoring Program Performance/ Adherence to Contract Measures &ndash; essential function: This position works to monitor performance in accordance with contractual measures and provides strategic direction to produce desired outcomes.
2.       Clinical consultation &ndash; essential function: This position will provide mental health consultation to the Care Coordinators and Peer Advocates working with for children, youth and adults in SBHE program.

Position Summary: The Family Allies Case Management Operations Specialist adheres to the Family Partnerships of Central Florida&rsquo;s System of Care principles of practice and provides family cantered strength-based assessment, planning, linkage, coordination, and advocacy for case management staff and families. This position provides direct support to case management team by completing needed assessments, responding immediately to requests from outside stakeholders, and assisting with quality and training to case management staff.  The Case Management Operations Specialist will also provide observed trends, opportunities for efficiency, and feedback to supervisors and directors and will act as the liaison for Care Managers with stakeholder requests for assistance or information, referrals, court reports, or other needed assistance.  The Case Management Operations Specialist works as a team member with key partners including case management staff, process servers, guardian ad litem, DCF Child Protective Investigators, CLS attorneys, therapists, providers, clients, and other appropriate professionals.  This position is expected to be an expert in Florida&rsquo;s child welfare system and build strong relationships and processes for the behind the scenes work that goes into case management so care managers can spend more time working directly with the children, parents and caregivers the agency serves.   
 
Must successfully maintain Child Protection Certification training and provide coaching and mentorship to multiple case management staff on agency processes and practices.  Will be regarded as a Qualified Evaluator to support Care Managers can required certification activities and maintain Human Trafficking certification. 
 
Technical/Functional Expectations:
 
1.       Complete required certification &ndash; Maintain CWCM certification with the Florida Certification Board.
2.       Safety - Conduct family function and child safety assessments; develop, monitor, and manage required safety plans.
3.       Permanency - Identify and develop plan to successfully achieve permanency for children placed in out-of-home care.

ESSENTIAL FUNCTIONS:





&middot;       Provide specialized assistance to Child Welfare teams and the children and families in care.
&middot;       Coordinate planning strategies and maintain positive relationships with agencies and         community members as applicable.

&middot;       Assist in the assessment of client&rsquo;s behaviors and emotional needs to determine the best possible community services to be implemented with/ for youth/ family.

&middot;       Staff cases for diversion/prevention involvement and or to implement a safety service as needed.
&middot;       Expected to be a part of the CPI initial consultations and 2nd  tier staffing&rsquo;s. Also available to be included in impromptu staffing regarding present and impending danger plans. Be a critical part in the decision of what services are available and can be implemented to ensure a child/ children is able to be kept in their home safely.
&middot;       Attach themselves to a CPI case that has been identified as needing prevention services and assist CPI with gathering information from parents/ other providers that the family may have been involved with or is currently with to move case forward and keeping the children safe without &ldquo;ongoing&rdquo; case management.
&middot;       Act as the conduit between providers and families. They are the linkage of services and ensure that contact between the provider and family has occurred and they are engaged. Also obtain all monthly documentation from providers and scan into the FSFN file cabinet and or update notes in FSFN timely and continuously.
&middot;       Ensure that parents sign the FSS Acknowledgement forms upon initial contact.
&middot;       Completes funding authorization form/ Diversion Supervisor signs the funding authorization and Diversion Manager approves and signs off for final approval.
&middot;       Screens all referrals to community providers and programs in the community.
&middot;       Maintain and complete ongoing Diversion tracking logs (weekly and as needed)

POSITION OBJECTIVE:  
The overall primary function of the Peer Support Specialist will include peer mentoring, advocacy, and connecting clients to resources. Using their lived experience, Peer Support Specialists will focus on mentoring those with a substance abuse disorder (SUD). This will involve building healthy relationships and trust and walking alongside individuals moving towards or currently involved in the recovery process. More specifically, peers can assist clients with exploring treatment options, facilitating communication with service providers, and helping develop realistic recovery goals tailored to each participant. Peers will share and utilize tools to support the recovery process. This will include the development of a personalized recovery action plan, designed to help manage challenges associated with recovery.

This role evaluates applications and provides recommendations for the approval or denial of licenses. The specialist also assesses incident reports and licensing concerns related to potential foster homes, offering recommendations for appropriate resolution. Under the guidance of the Licensing Supervisor, the Licensing Specialist is responsible for collecting, organizing, monitoring, and distributing documentation to ensure licensing standards are met. This position supports the successful progression through the various levels of initial or re-licensure for foster care applicants, ensuring compliance with all regulatory requirements.
 
Technical/Functional Expectations
1.       Complete Licensing Home Studies-Essential Function: The position completes initial and/or re-licensure home studies inclusive of the assessment and evaluation of all household members and babysitters, to include background screening and abuse report history. 
2.       Background Checks &ndash; Essential Function: Review and evaluate criminal history reports, fingerprint results, child abuse and neglect registry checks, sex offender registry searches, and other required clearances. Ensures continuous compliance with Title IV-E requirements by monitoring and tracking background check renewals, preventing lapses in required screenings and clearances for foster parents and household members. 
3.       Home Visits to Family Foster Homes-Essential Function: This position will provide regular and ongoing home visits to family foster homes throughout the licensure process.
4.       Conduct Ongoing Assessments-Essential Function: This position will conduct ongoing assessments to determine capacity as indicated based on family skills, training, household composition, physical space and other determining factors.
5.       Liaison between Foster Parents and Case Managers &ndash; Essential Function: This position will serve as a Liaison between foster parents and case managers ensuring open communication and timely resolution of any identified concerns when applicable.
6.       Foster Care Referrals-Essential Function: This position will comply with Administrative Code requirements for any foster care referral on a licensed foster home. The licensing specialist will make recommendations for resolution of
all incident reports, foster care referrals and licensing concerns related to existing foster homes. This includes any joint visits for DCF institutional reports on assigned foster homes.

Maintain open communication with the Youth Services Team and other community partners to connect youth and young adults that need assistance in specified areas.

Participate in the Agency&rsquo;s Quality Assurance and Improvement activities deployed throughout the organization to include contracted providers including technical assistance, training, and assistance in policy development as needed.

Position Summary: The Licensing Specialist serves as the primary point of contact for prospective foster parents and is responsible for the timely and thorough completion of foster home studies in accordance with Florida Administrative Code. This role evaluates applications and provides recommendations for the approval or denial of licenses. The specialist also assesses incident reports and licensing concerns related to potential foster homes, offering recommendations for appropriate resolution. Under the guidance of the Licensing Supervisor, the Licensing Specialist is responsible for collecting, organizing, monitoring, and distributing documentation to ensure licensing standards are met. This position supports the successful progression through the various levels of initial or re-licensure for foster care applicants, ensuring compliance with all regulatory requirements.
 
Technical/Functional Expectations
1.       Complete Licensing Home Studies-Essential Function: The position completes initial and/or re-licensure home studies inclusive of the assessment and evaluation of all household members and babysitters, to include background screening and abuse report history. 
2.       Background Checks &ndash; Essential Function: Review and evaluate criminal history reports, fingerprint results, child abuse and neglect registry checks, sex offender registry searches, and other required clearances. Ensures continuous compliance with Title IV-E requirements by monitoring and tracking background check renewals, preventing lapses in required screenings and clearances for foster parents and household members. 
3.       Home Visits to Family Foster Homes-Essential Function: This position will provide regular and ongoing home visits to family foster homes throughout the licensure process.
4.       Conduct Ongoing Assessments-Essential Function: This position will conduct ongoing assessments to determine capacity as indicated based on family skills, training, household composition, physical space and other determining factors.
5.       Liaison between Foster Parents and Case Managers &ndash; Essential Function: This position will serve as a Liaison between foster parents and case managers ensuring open communication and timely resolution of any identified concerns when applicable.
6.       Foster Care Referrals-Essential Function: This position will comply with Administrative Code requirements for any foster care referral on a licensed foster home. The licensing specialist will make recommendations for resolution of
all incident reports, foster care referrals and licensing concerns related to existing foster homes. This includes any joint visits for DCF institutional reports on assigned foster homes. 
7.       Confidentiality &ndash; Essential Function: Maintain strict confidentiality and security of sensitive personal information in compliance with applicable privacy laws and regulations. 
8.       Licensing Packets &ndash; Essential Function: Ensure licensing packets for caregivers meet code requirements, licensing expiration and DCF submission requirements.
9.       Foster Care Regulations &ndash; Essential Function: Stay current on changes to foster care licensing regulations, background screening requirements, and agency procedures. 
10.    Knowledge of Microsoft Office Applications &ndash; Essential Function: Utilize Microsoft Office applications and agency databases to manage workflow, analyze information, and produce reports. 
11.    Attention to Detail and Judgement &ndash; Essential Function: Demonstrate strong attention to detail, critical thinking, and sound judgment when reviewing eligibility-related information. Effectively prioritize workload and manage multiple cases while meeting established deadlines.
12. On Call Rotation &ndash; Essential Function: Participate in first tier on call rotation. 
13. Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting, and training others, and performing duties of higher rated positions from time to time for developmental purposes.

1.       Valid Driver License - Essential: This is required to travel throughout the County and other locations for client and other meetings.

3.       Advanced computer and data entry skills: This is required to maintain spreadsheets and data in Level 1 licensing.
4.       Collaboration, negotiation, and problem-solving skills: This skill and experience is required to ensure successful integration into our system of care.
 
Educational and Experience Requirements:
1.           An associate degree from an accredited college or 60+ college credits from an accredited college AND two years of full-time social work or human services experience or two years of professional work experience. OR a high school diploma or GED equivalent (official transcripts required) AND four or more years of full-time social work or human services experience.
2.           BA/BS - Bachelor&rsquo;s Degree or equivalent preferred.
3.           2+ years licensing experience preferred.
4.           Florida Child Welfare Certification required (training provided)

      <description><![CDATA[Position Summary: The Family Allies Care Manager adheres to the BFP System of Care principles of practice and provides family cantered strength based assessment, planning, linkage, coordination, and advocacy for children and families. The Care Manager works as a team member with key partners including parents, family members, foster parents, caregivers, guardian ad litem, DCF Protective Investigators, CLS attorneys, therapists, providers, and other appropriate professionals. The Care Manager will train and become proficient in identifying and assessing child and family needs with the ultimate goal of achieving permanency.
Must successfully complete Child Protection Certification training and examination, then a considerable period of monitored field practice under close supervision and the associated field case assessment process.
Technical/Functional Expectations:

Complete required certification - essential function: Attend Child Protection Certification training (pre-service) and successfully attain a passing score of 86% or higher on the required examination.
Safety - essential function: Conduct family function and child safety assessments; develop, monitor, and manage required safety plans.
Permanency - essential function: Identify and develop plan to successfully achieve permanency for children placed in out-of-home care.
Service Delivery - essential function: Assess child and family needs; develop service plans; coordinate the delivery of services; ensure all referrals and linkages to children/parents/caregivers are completed.

Support - essential function: Provide support to children, caregivers and family members to assure stable placement and timely achievement of permanency; Coordinate cases with supervisor, team members, providers, DCF and attorneys as needed.
Court Appearance and Preparation - essential function: Review case information prior to every hearing in order to be prepared to answer any questions from the Court regarding the status of the case, services, and progress towards permanency; Dress appropriately for Court appearances and ensure all court hearings are attended timely.
Initiatives - essential function: Embraces and participates in initiatives and programs designed to enhance the System of Care.
On Call - essential function: This position is required to participate in the on-call function on nights and weekends.

Master's degree in Social Work or related field of study preferred
