JOB FUNCTION:  
FaithBridge Youth Opportunities is a program assisting former foster and homeless youth to achieve their dreams for the future through vocational training, financial assistance, and supportive care. Youth Opportunities operates under FaithBridge Foster Care, Georgia&rsquo;s largest Christ-centered foster care agency (a non-profit organization) located in metro Atlanta. 

At the heart of the Youth Opportunities Program is the role of the Life Coach. The Youth Opportunities Life Coach is our unique version of the case management role serving youth who have aged out of foster care or who may be homeless. A Life Coach works closely in support of the needs of youth between the ages of 17 and 26 who are pursuing their post-secondary education or vocational training. The Life Coach assists in managing their intake and admission to FaithBridge Youth Opportunities. The Life Coach walks alongside the youth as they seek to enroll in school, obtain vocational training, seek employment, gain necessary life skills, and begin to live life as a successful adult. 
 
A Life Coach also is responsible for administering and ensuring the youth&rsquo;s monthly support needs are met and documented in the appropriate data system. They help ensure each youth&rsquo;s safety and well-being through weekly meetings and frequent check-ins with youth   Life Coaches promote youth stability through coaching,  service coordination, spiritual guidance, and crisis intervention. Life Coaches advocate for each youth&rsquo;s best interest by attending meetings related to the youth&rsquo;s care or case.
 
Application REQUIREMENTS:  Resume, Cover Letter and Statement of Faith required to be considered for position.  Prefer candidates who live in the counties of youth caseload.
 
Core Responsibilities and Expectations
 
o    Maintains and abides by all FaithBridge policies and procedures.
o    Advocates for the best interest of youth vocation and education goals as well as their housing, well-being, and safety.
o    Assess and ensures youth receive timely and appropriate services as needed such as medical, dental, educational, mental health, social and behavioral counseling. 
o    Coordinates and assists youth with transportation needs.
o    Monitors the need for and facilitates discharge planning when appropriate.
o    Communicates with Youth Opportunities team regarding progress and direction of case as appropriate.
o    Supports a healthy relationship with the youth&rsquo;s prior foster family and birth family and other supportive adults. 
o    Knowledge of de-escalation techniques. Assists youth to manage periodic disruptions in their plans and is sensitive to youth their needs while in the community.
o    Assesses the impact of youth living independently and provides necessary intervention and support as needed.
o    Provide Christian support and Faith Based mentorship to youth.
o    Utilizes clinical and consulting skills to provide coaching and support to youth. 
o    Ability to recognize a client for self-harm/suicide/homicide risk and make appropriate referrals, seek the support of the leadership team, and follow safety plans. 
o   Demonstrates a working knowledge of trauma and how that impacts youth.
o    Assist youth in securing appropriate housing, employment, and financial stability including effective budgeting including increasing contributions to their savings account.
o    Responsible to help approve reimbursement requests from youth related to their living needs and provide necessary documentation to supervisor and accounting department. 
o    Communicate and collaborate with the youth&rsquo;s team members&rsquo; including those in the school, extended foster care case managers, service providers, etc., in support of their needs to support their progress on their goals.
o    Demonstrates ability to navigate high stress situations and conflict resolution using emotional and spiritual maturity.
o    Completes required documentation, including service plans, assessments, progress notes, financial reconciliations etc. within the required timeframes.
o    Works in collaboration with church partners and the Community of Care to meet the needs of the youth in Youth Opportunities care. 
o    Communicates with Ministry Team Leaders when youth are placed, moved, have any incidents, or need support or volunteers.
 
 
EQUIPMENT USED:  FaithBridge provided cell phone and laptop, access to office printer/copier. It is expected that staff utilize a personal vehicle with mileage reimbursement.
 
QUALIFICATIONS:
Education and Experience: Degree in Human Services Field (i.e.., Social Work, Psychology, Behavioral Science, Counseling, Ministry/Theology, or other related degree). MA/MS preferred. BA/BS required. Clinical Experience: Strong experience working with youth over 18 with some clinical knowledge, particularly in the areas of trauma, family-systems, substance abuse and life skills. Strong coaching/counseling and crisis intervention skills.  Experience with the foster care system preferred, minimum 2-3 years case management experience.   Preferably experience working with teens or young adults, DFCS is a plus. 
 
Skill Requirements: Strong interpersonal skills, maturity and sensitivity to cultural and individual differences in clients, staff, and the communities in which we serve.  Effective oral and written communication skills.  Ability to work within a team.  Ability to handle job related matters in a professional, diplomatic, and confidential manner.  Knowledge of community resources and services. Ability to organize and execute responsibilities in an independent manner. Knowledge of normal business computer skills. Competent in the use of MS Word, Excel, PowerPoint. Experience working with case management software desired.
 
Physical Requirements: Hearing and speaking ability which allows for effective oral communication of information. Ability to physically transport self and clients to various locations. Physical and emotional stamina to effectively handle job related issues and stress.]]></description>

FaithBridge Foster Care, Georgia&rsquo;s largest Christ-centered foster care agency (a non-profit organization) located in metro Atlanta, is hiring a Foster Family Consultant serving Henry, Dekalb and South Fulton Counties
 
The Foster Family Consultant (FFC) is our unique version of the case management role within a child placing agency. FFCs work closely with DFCS and the juvenile court system to manage children&rsquo;s dependency cases toward permanency. They participate in matching foster children with foster families during the placement process. They ensure each child&rsquo;s safety, permanency, and well-being while in foster care through home visits and frequent involvement with the child, foster family, and birth family. FFCs support birth family reunification efforts through birth parent visitations and intentional birth parent support. FFCs promote foster placement stability through foster parent coaching, 24/7 support, service coordination, spiritual guidance, treatment planning, and crisis intervention. FFCs advocate for each child&rsquo;s best interest by attending all court hearings, panel reviews, family team meetings, educational meetings, and other meetings related to the child&rsquo;s care or case.
 
Core Responsibilities and Expectations
 
o   Maintains and abides by all court orders
o   Supports a goal that has children and families&rsquo; best interest at all times
o   Is an active advocate for children and families
o   Participates in on-call rotation as needed
o   Assess and ensures children and families receive timely and appropriate services such as medical, dental, educational, mental health, social and behavioral
o   Coordinates and/or participates in court hearings, family team meetings, home visits, panel reviews, birth family contacts, educational advocacy, permanency round tables, clinical staffing&rsquo;s and other meetings as needed
o   Monitors the need for and facilitates discharge planning when appropriate
o   Assists and supports permanency planning for children who are unable to be reunified
o   Communicates with home study team any foster home licensing needs, concerns, changes, and ministry re-engagement plans
o   Communicates with all service providers regarding progress and direction of case as appropriate
o   Supports birth family relationships as appropriate
o   Coordination of visitation with birth family
o   Manages disruption and is sensitive to birth and foster family&rsquo;s needs
o   Understands and supports family systems
o   Assesses the impact of fostering on the foster family system and provides necessary intervention and support as needed
o   Provide pastoral care to children, foster and birth families
o   Provide 24/7 care as needed to meet foster children and foster family needs
o   Utilizes clinical and consulting skills to provide coaching and support to families
o   Ability to recognize a client for self-harm/suicide/homicide risk and make appropriate referrals
o   Demonstrates a working knowledge of trauma and how that impacts the children and families served.
o   Demonstrates authentic, honest and effective communication skills
o   Ability to intervene and provide clinical skills, crisis intervention, and mediation
o   Knowledge of de-escalation techniques
o   Provides post-placement & pre-placement visits with foster families to promote optimum utilization
o   Manages utilization of foster homes through consulting and pastoral care with foster and respite families
o   Manages foster and respite homes yearly training hours and documentation to maintain licensing requirements.
o   Responsible to approve reimbursement requests from foster parents related to the child and submit to accounting department.
o   Works and collaborates with individuals including DFCS, CMS, GAL, child attorneys, service providers, etc. on their cases to move them toward permanency
o   Demonstrates ability to navigate high stress situations and conflict using emotional and spiritual maturity
o   Completes required service plans, assessments, other documents and monthly contacts according to required timeframes
o   Works diligently to be well-informed regarding a child&rsquo;s background and needs
o   Advocates strongly on issues that are important to the child&rsquo;s well-being, safety and permanency
o   Demonstrates an ability to positively navigate transition and change and models flexibility for others
o   Maintains relationships with church partners as it relates to FaithBridge foster families
o   Works in collaboration with church partners and the Community of Care to meet the needs of the children and families
o   Communicates to Ministry Team Leaders when children are placed, moved, in need of respite and/or volunteers

QUALIFICATIONS:
Education and Experience: Degree in Human Services Field (ie., Social Work, Psychology, Behavioral Science, Counseling, Ministry/Theology, or other related degree). MA/MS preferred. BA/BS required. Clinical Experience: Strong clinical knowledge, particularly in the areas of childhood trauma, family-systems, substance abuse. Strong coaching/counseling and crisis intervention skills. Experience with GA Score and Shines preferred, but not required. Georgia RBWO Foundations Certified or waiver eligible is beneficial, but not required.
