Conduct weekly home visits and maintain an assigned caseload  

Administer screenings and assessments; provide referrals as needed  

Deliver education on parenting, child health, and home safety  

Build strong, culturally responsive relationships with families  

Support outreach, recruitment, and community engagement efforts  

Assist with program promotion, especially in Wards 5, 7, and 8  

Maintain accurate and timely data in required systems  

Participate in trainings, meetings, and professional development  

1+ year of experience in social services, public health, education, or related field (or lived experience)  

Associate or bachelor's degree in public health, psychology, education, social services, or related field (preferred)  

Experience working with children and families strongly preferred  

Strong communication and interpersonal skills  

Ability to build trust with diverse communities  

Organizational and time management skills  

Experience with assessments, referrals, and case management  

Basic data entry and documentation skills  

Field-based role requiring travel to clients’ homes, community sites, and events  

Combination of remote, office, and in-community work  

May require occasional evening or flexible hours  

Contract through September 30, 2026 (with potential extension based on funding)
