The Nurse Case Manager would be responsible for tracking the medical status of Service Members (SM). The incumbent would support SM in becoming Fully Medically Ready (FMR) by tracking medical and dental appointments as well as by coordinating treatment plans and all referrals when the care is being resourced by the Government.
Required Qualifications:
• A minimum of one year of experience in the practice of nursing.
• A minimum of one year of experience in case management knowledge.
• Clear background check.
• Previous experience with the army is preferred but not mandatory.
