Dementia Care Coordinator: Seeking a Dementia Care Coordinator [DCC]. The DCC is responsible for the overall management and success of the dementia (Reflections) neighborhood. The responsibilities include ensuring excellence in resident care and programming, providing outreach and support to families, hiring and retaining dedicated care managers, hands on care to a dementia population, and facilitating ongoing training and educational opportunities for staff and families.  This dementia leader ensures that the designated care manager concept is implemented.  The DCC is expected to be knowledgeable as to the dementia process and to be constantly learning about the most current information on Alzheimer's disease.  In this position of leadership, the coordinator will create pleasant days for the residents in an atmosphere of warmth, comfort, security, and joyfulness.

Qualifications:

• Supervisory and management experience
• Able to make responsible choices and decisions and act in a resident's best interest
• Minimum of one (2) year of experience in assisted living or long-term care
• Experience with a dementia population 
• Nursing experience preferred
• Exhibits a caring heart with a passion to care for an elderly population
• Good clinical assessment skills
• Experience using healthcare technology
• Good organizational and time management
• Able to work flex hours and be available to respond to community during off-duty hours regarding clinical concerns and emergencies
• Possesses good judgement, problem solving, and decision-making skills
• Possesses the ability to effectively read, write and communicate
