    Thompson is seeking a Case Manager to join our Thompson RISE Team.  

    Thompson RISE is a one-on-one community-based coaching service for young adults ages 16-21 to help provide them with the tools and knowledge they need to successfully transition to adulthood. Each youth is assigned a Coach that works with them weekly to achieve their goals whether it be securing gainful employment, securing and stabilizing their housing situation, learning how to manage money, and academic pursuits.

    ​Job Responsibilities:

    • Display individual proficiency and effectiveness in the role

    • Represent the organization internally and externally through your communication and conduct
    • Display trauma informed practices and principles in your interactions with customers, with peers and with leadership
    • Provide behavior management, case management and general care coordination for assigned youth
    • Provide effective crisis response and crisis management for assigned youth
    • Provide direct support to your clients in the following four core domains: education, employment, securing stable housing and avoiding legal involvement.
    • Achieve productivity expectations which align with clients and families receiving their necessary treatment
    • Ensure documentation standards which adhere strictly to policy and procedure and meet all regulatory and billing standards
    • Other duties as assigned
    Position Specifications and Education: 
    • Must have a valid Driver License and meet any credentialing, licensing, and privileging standards as it pertains to the department you are in.
    • Education requirement: HS Diploma or GED
    • A minimum of 2 years of relevant professional experience
    • Proficient in Microsoft Office Suite application software, Excellent written and oral communication skills
