SUMMARY: This position is responsible for providing overall leadership and management of the health and well-being of the residents within the community. This position is required to work every other weekend.

 

DUTIES AND RESPONSIBILITIES:

•      Maintains communications with Healthcare Providers of recent changes or requests for orders.

•      Ensures that community follows all federal, state, local laws and regulations as it pertains to clinical care.

•      Collects, and reports occurrences, and if necessary, assist in developing/implementing improvement plans.

•      Partners with Administrator and other team members to maintain resident and team member health safety.

•      Partners with pharmacy consultant to provide optimal pharmaceutical services to residents.

•      Oversees infection control practices in community.

•      Oversees the safe and efficient distribution/administration of medications, injections, nebulizers, moderate wound care per state regulations.

•      Identifies ongoing needs and services of residents through the evaluation/ Service Plan process including documentation for residents with a change in health care status.

•      Manages the completion of any assigned evaluations.

•      Partners with other department coordinators to identify, review, and discuss potential change in service/medication levels and updates the resident’s service plan as indicated by state regulations.

•      Maintains communications with resident’s family and/or responsible party regarding changes in care or health concerns.

•      Performs other related duties as assigned by management.

 

SUPERVISORY RESPONSIBILITIES:

·        Directly supervises certified nursing assistants’ employees within the department(s).

  • Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.

 

QUALIFICATIONS:

·        Associate's Degree (AA) or equivalent from a two-year college or technical school, or six months to one year related experience and/or training, or equivalent combination of education and experience.

·        Certificates, licenses and registrations required:

o   Maintains current state license as a Licensed Professional Nurse or a Professional Registered Nurse per state regulations.

·        Other skills required:

o   Experience in assisted living, home health or long-term care industries.

o   Two (2) years’ experience as a Licensed Nurse.

o   Must demonstrate competence in assessment skills, injections/medication administration, follow up and triage.

o   Ability to communicate effectively speaking the primary language of the residents.

o   Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.

o   Ability to write routine reports and correspondence.

o   Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.

o   Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form; ability to deal with problems involving several concrete variables in standardized situations.

o   Proficiency in computer skills, Microsoft Office and ability to learn new applications.
