Purpose: 
A patient navigator guides patients through the confusing maze of health care with caring, sensitivity and compassion. (S)he can focus exclusively on the needs of patients and help resolve concerns about the quality of care so they can feel all their needs are being met.   This position will work with multiple departments inside and outside the organization to address the patient’s needs and concerns.


Responsibilities/Essential Functions (other duties as assigned):
 
  • Performs assessment of patient care needs for new and ongoing patients.
  • Work with physician and clinical office to facilitate, coordinate and ensure patient has access to available resources.
  • Serve as an essential link between patients and all other providers.  Central point of contact for all treatment patients; including interaction with medical, nursing, ancillary services.
  • Collect and report data from patient on barriers and intervene as appropriate for optimal patient care.
  • Assist patients with referrals to outside resources and care coordination as needed.
  • Follows established departmental policies, procedures and continuous quality improvement objectives.
  • Collaborates with medical providers, patient care staff and clinic management in the planning and implementation of patient and staff education.
  • Designs or participates in the creation of educational materials.
  • Performs miscellaneous job-related duties as assigned.

Experience, Qualifications/Education:
  • High school diploma or equivalent required
  • Graduate of an accredited School of Nursing ( RN/LVN ).

  • Strong customer service background, preferably in healthcare environment
  • Excellent verbal communication skills
  • Ability to manage and prioritize multiple tasks.
  • Proficiency in computer processing functions and other standard office equipment
  • Ability to calmly and professionally resolve customer issues with diplomacy and tact
  • Ability to work independently with minimal supervision.
  • Strong organizational skills

Working Conditions:
 
  • This position functions primarily indoors in a medical / business environment. Employee may be subject to blood borne pathogens, radiation, bio waste and other medically-related factors, with exposure to moderate noise levels.
  • Visual acuity to read and compute screen and paper documents; close and distance vision, peripheral vision depth perception, ability to adjust focus; hearing acuity to converse with staff and customers; ability to walk, stoop, bend, kneel, and lift and move 25 pounds on a regular basis; ability to sit for hours at a time. Employee will be exposed to moderate noise levels, interruptions, and occasionally asked to work evening and weekends as needed.
