POSITION: PROGRAM SPECIALIST - # 98-02
HOURS: 37.5 HOURS PER WEEK, 1ST SHIFT, MUST BE FLEXIBLE
LOCATION: LIFE SHARING – ERIE COUNTY
AVAILABLE: IMMEDIATELY
REQUIREMENTS: Responsibilities include planning, organizing, and directing provisions of cross-disciplinary services to clients in a community based program for intellectually disabled individuals. Responsibilities also include planning or participation in planning, developing, and evaluating therapeutic care and treatment programs. Will also act as team facilitator in Interdisciplinary Team meetings to develop Individual Support Plans (ISP’s) on each assigned client.
Requirements include a Bachelor’s Degree and two (2) years of experience working with individuals with intellectual disabilities, OR an Associate’s Degree or 60 credit hours from an accredited college or university and four (4) years of experience, OR a Master’s Degree and one (1) year of experience. Supervisory experience is a must. Must understand HCSIS systems. Knowledge of ODP programs is beneficial.
