Purpose: 
A nurse/patient navigator is defined as a medical professional whose clinical expertise and training guides patients and their caregivers to make informed decisions, collaborating with a multidisciplinary team to allow for timely cancer screening, diagnosis, treatment, and increased supportive care across the cancer continuum. In essence, nurse navigators guide patients through the healthcare system and help them to overcome barriers to optimal care. The navigation of patients with cancer requires extensive knowledge, specialized skills, and good clinical judgment, which can improve outcomes. This position will work in coordination with St. Agnes Medical Center and cCARE.

Responsibilities/Essential Functions (other duties as assigned):
  • Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients.
  • Serve as an essential link between patients and all other providers.  Central point of contact for all navigated patients; including interaction with medical, nursing, ancillary services, and when appropriate the clinical research coordinator. Identify potential patients for Clinical Trials protocols and coordinates with the research coordinator to consent the patient.
  • Provides education to patients, families, and significant others; acts as an information resource to students, health care professionals, patients, and the public.
  • Collect and report data from patient on barriers, health care, medications, prior treatments, etc. Identify barriers to the plan of care and intervene as appropriate to provide optimal patient care.
 
  • Track appointments and follow up for surgery, medical oncology and radiation oncology.
  • Assist patients with referrals to outside resources and care coordination as needed.
  • May attend and refer patients to tumor board and follow up on tumor board recommendations.
  • May participate in the orientation, training and evaluation of new staff.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives and safety, environmental, and/or infection control standards.
  • Collaborates with medical providers, patient care staff and clinic management in the planning and implementation of patient and staff education.  Assists in managing overall patient flow of the Specialty Clinic.
  • Designs or participates in the creation of health education materials.
  • Performs miscellaneous job-related duties as assigned.


Experience, Qualifications/Education:
  • High school diploma or equivalent required
  • Graduate of an accredited School of Nursing
  • Current California Registered Nurse License and CPR certification required
  • Minimum of 3 years Oncology nursing experience required.
  • Previous Breast/Patient Navigator experience required.
  • 6 years or greater of RN experience preferred.
  • OCN certification strongly preferred if not required.
  • Bachelors in nursing strongly preferred if not required.
  • Strong customer service background, preferably in healthcare environment
  • Excellent verbal communication skills
  • Knowledge of drug names, both brand and generic, and available dosage forms
  • Ability to manage and prioritize multiple tasks
  • Proficiency in computer processing functions and other standard office equipment
  • Ability to calmly and professionally resolve customer issues with diplomacy and tact
  • Ability to work independently with minimal supervision
  • Strong organizational skills

Working Conditions:
 
  • This position functions primarily indoors in a medical / business environment. Employee may be subject to blood borne pathogens, radiation, bio waste and other medically-related factors, with exposure to moderate noise levels.
  • Visual acuity to read and compute screen and paper documents; close and distance vision, peripheral vision depth perception, ability to adjust focus; hearing acuity to converse with staff and customers; ability to walk, stoop, bend, kneel, and lift and move 25 pounds on a regular basis; ability to sit for hours at a time. Employee will be exposed to moderate noise levels, interruptions, and occasionally asked to work evening and weekends as needed.
