The Hospice Social Worker uses experience, knowledge, judgment and skills to provide social services to patients and families in the hospice setting. The Social Worker maintains patient confidentiality and promotes patient rights as supported by agency, state and federal directives. Identifies cultural diversity and lifestyle choices and non-judgmentally accommodates for such differences. Additionally, they provide consultation to other hospice staff and volunteers while consistently demonstrating hospice values and compliance with organization’s policies and procedures. The Hospice Social Worker coordinates and delivers a service program designed to meet the psycho, social, emotional needs of the patient and family, helping them to adjust to the changes related to their illness and related end-of-life care planning.Knowledge, Skills, and Abilities
Education: Master’s Degree in Social Work from an institution accredited by the Council on Social Work Education
Experience: A minimum of one year of experience as a social worker in a health care setting, preferably hospice, home health, or hospital setting.
Licensure/Certification: Current North Carolina MSW licensure. Must have a current CPR certification.

Knowledge/Skills: Demonstrates an understanding of the hospice philosophy. Comfortable dealing with suffering, death and dying patients and their family members. Basic computer skills and the ability to become proficient in company software and data systems. Demonstrates the ability to work within an interdisciplinary team setting. Excellent communication skills both verbal and written.
Contacts: Community agencies and businesses, agency personnel, patient/families, physicians, and various medical field personnel.
Physical Demands: Frequent sitting. Frequently requires long periods of working at a computer. Intermittent walking, standing, stooping, bending, stretching. May require lifting or moving items up to 25 lbs. Frequent use of finger/hand dexterity. Constant talking or hearing. Corrected hearing and vision to normal range. No exposure to blood, body fluids or tissue. Possible exposure to infectious materials, communicable diseases and/or other conditions common to a healthcare environment. Normal office environment. Requires work under stressful conditions, deadlines, and/or irregular hours. Ability to read and comprehend. Travel between facilities may be required.
