Description

The CPCC Nurse Navigator will work collaboratively with the urology healthcare team to assess, plan, facilitate, track and monitor patients with prostate cancer. The Nurse Navigator works closely with patients and family; makes appropriate referrals to meet the patient’s individual, cultural and emotional needs, and monitors and evaluates services provided. The Nurse Navigator works as a liaison to and in collaboration with patients, physicians, disease management liaisons, and other health care providers.

DUTIES AND RESPONSIBILITIES

  • Implement established UA prostate cancer protocols

  • Continue development and improvement of cancer care protocols in collaboration with the CPCC

  • Serve as patient navigator for all prostate including being a primary contact for all CPCC patients.

  • Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate CPCP therapies. Develop and distribute educational materials for patient use

  • Serve as a resource for community support group collaboration

  • Assist UA providers and patients in acquiring and completing enrollment forms for CPCC therapies

  • Identify benefits and coordinate resources based upon patient’s needs, insurance coverage and community programs

  • Serve as pharmacy technician for in office dispensing pharmacy program

  • Coordinate and administer Provenge therapy-ensure all pre-therapy paperwork and labs completed, make arrangements for apheresis, ordering drug, etc. 

  • Follow through with patient’s medication treatment plan, ensuring they have received education and medication as prescribed

  • Ensure that the patient and family understand the diagnosis processes, treatment strategy and recommended care plan, and are provided with appropriate information in coordination with the providers. Respond to patient requests for information regarding the disease process, expected side effects and community resources

  • Support the patient and family during difficult decision-making periods

  • Serve as an essential link between patients and all other care providers. Facilitate appointments for consults and support services, including nutritionist, oncologist, physical therapist, etc. 

  • Identify barriers to the plan of care and intervene as appropriate to provide optimal patient care

  • Responsible for tracking all prostate cancer patients to ensure patients are receiving all appropriate medical care, imaging, prescription refills, referrals- using PPS analytics

  • Other duties as assigned

EDUCATION AND WORK EXPERIENCE:

  • Bachelor of Science in Nursing with a minimum of 3 years experience, preferably in oncology and/or urology

  • Current CO RN license

QUALIFICATIONS & REQUIREMENTS:

  • Ability to assess, plan, implement and evaluate nursing care according to individual needs

  • Interest in providing cancer care case management as well as direct patient care

  • Must demonstrate consistent professional conduct and meticulous attention to detail

  • Must possess excellent verbal and written communication skills as well as excellent interpersonal skills with patients, staff, and other health care professionals

  • Requires excellent organizational skills and clinical knowledge

  • Familiarity with standard computer programs and EMR systems
