Sanitas is hiring a Full-Time Community Health Worker for our Cape Coral - HUB  facility in Cape Coral, FL. The Community Health Worker (CHW) serves as a vital link between Sanitas and the surrounding community. In this role, you will connect with local resources, map the community, and link leaders and services to our users.

Responsibilities:

• Identify local community resources by engaging with organizations and leaders.
• Collaborate with local organizations to create and promote community activities.
• Facilitate self-sustained groups, identifying community leaders.
• Connect residents to resources through community connectors.
• Support high-need patients by linking them to appropriate resources.
• Collaborate with the medical center team and participate in weekly meetings.
• Report to the Community Coordinator in Florida.

Skills & Qualifications:

• High school diploma or equivalent; certification as a Community Health Worker in Florida required.
• 1 year of experience in community development and/or outreach.
• Knowledge of local community needs and effective information gathering.
• Strong public speaking and administrative skills.
• Proficiency in Microsoft Office Suite required.
• Bilingual in Spanish and English required.

Physical Demands: This role requires regular physical activity, including standing, walking, and lifting up to 50 pounds, with specific vision abilities and exposure to various environmental conditions.
