Job Summary: We are seeking a highly skilled and compassionate Dual Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP) and Family Nurse Practitioner (FNP) to join our team. This role will involve providing psychiatric consults and chronic care management. The ideal candidate will have a strong background in both psychiatric and family health, excellent communication skills, and a passion for improving patient outcomes.
Key Responsibilities:- Conduct comprehensive psychiatric evaluations and provide ongoing psychiatric care and management for patients/residents.
- Develop and implement individualized treatment plans for patients with mental health and substance use disorders.
- Provide medication management.
- Collaborate with a multidisciplinary team, including physicians, therapists, and social workers, to ensure coordinated and comprehensive care.
- Conduct chronic care management for residents in a senior living community, focusing on the management of chronic diseases such as diabetes, hypertension, and heart disease.
- Perform routine health assessments, diagnose and treat common health conditions, and provide preventive care.
- Educate patients and their families about chronic disease management and healthy lifestyle choices.
- Maintain accurate and up-to-date medical records in compliance with regulatory standards and company policies.
- Dual licensure as a Psychiatric Mental Health Nurse Practitioner (PMHNP) and Family Nurse Practitioner (FNP).
- Master's or Doctoral degree in Nursing from an accredited institution.
- Current and valid Louisiana licensure as a Nurse Practitioner.
- Certification from a recognized credentialing body (e.g., ANCC, AANP).
- Minimum of two years of experience in psychiatric and family practice settings.
- Strong clinical skills in psychiatric assessment, diagnosis, and treatment, as well as chronic care management.
- Excellent interpersonal and communication skills, with the ability to work effectively as part of a multidisciplinary team.
- Compassionate and patient-centered approach to care.
- Proficiency in electronic health records (EHR) systems.

- $150,000.00 Annually to $165,000.00 Annually
